Supervisory Responsibilities:



Summary: This position provides direct case management services to the clients who seek services and general information regarding their needs.

Essential Duties and Responsibilities include the following. Other duties may be assigned.

  • Provide crisis intervention to individuals and families who reside in the appropriate community district.
  • Conduct thorough needs assessment and develop a service plan to address identified, concrete needs using a short-term intervention model.
  • Conduct assessments of individual and family problems and follow-up with referrals to Catholic Charities programs and/or clinics or an outside facility as deemed appropriate.
  • Maintain current and accurate documentation of services provided to clients, pursuant to CCCS requirements.
  • Process rental arrears grants by obtaining supporting eligibility documentation. Provide short-term case management focusing on assessing factors precipitating housing crisis. Develop and implement preventative measures to reduce risk of eviction in the immediate future.
  • Attend regular supervisory sessions with immediate supervisor.
  • Attend Agency' s in-service training and unit meetings as well as any other agency-related activities.
  • Complete daily, weekly and monthly statistical reports.
  • Develop and maintain good communication with other programs and departments within and outside the organization.
  • Promote and support Cultural Center activities.

Position Type and Expected Hours of Work:

This is a part-time position Monday - Friday. Evening and occasional Saturday hours to meet program deadlines, or client needs, and support special events.

  • Ability to work with the public and/or with clients in stressful situations.
  • Remain stationary at a workstation and use a computer at least 75% of the time.


Education and/or experience required:

  • Bachelor's degree or at least two years' experience in case management.
  • Bilingual Spanish/English a plus. Excellent interpersonal skills.

Skills, Licenses, and/or competencies required:

  • Good organizational, time management and communication skills.
  • Must possess the ability to work well independently as well as part of a team.
