Summary: The Feeding Our Neighbors Department provides emergency meals to individuals and families who

deal with hunger and food insecurity across the Archdiocese of NY. Our goals is to provide healthy nutritious

meals to anyone who is in need with compassion and dignity. This position will serve as a vital link between clients seeking emergency food and other supportive services that foster family stability and a healthy living. This position will work with clients to access and obtain supportive services, referral resources and pubic benefits including SNAP. The Case Manager will collaborate with local community-based organization, CC affiliated agencies, CCCS programs and/or government agencies to achieve goals for self/family sufficiency.

Essential Duties and Responsibilities include the following. Other duties may be assigned.

  • Conducts thorough assessment of individual and family who come into our food programs with general social service needs. Clients will be evaluated on their needs beyond the presenting issue and will be connected with on-going services that will provide short term and long-term support.
  • Assist individuals and families who come into our food programs with SNAP enrollment and other public benefits. In particular, use comprehensive and up-to-date information and resources about the SNAP program to facilitate enrollment, re-certification, and on-going participation in the program.
  • Provide information and referrals of useful resources which include any government programs, community-based organizations, CC affiliated agencies, and CCCS programs.
  • Develop and maintain collaborative relationships with government programs, community-based organizations, CC affiliated agencies and CCCS programs. Assist with food pantry/food hub services as needed or required by supervisor.
  • Conduct follow up with clients to ensure that stability and goals have been obtained and/or that additional assistance is needed.
  • Enter all client information into appropriate database(s) as required by program or funding source(s). Ensure that contract or program deliverables are met and achieved as required.
  • Maintain good record keeping, documentation and maintain good communication with all external and internal personnel to ensure efficiency in work and caseload management. Conduct case review of files with supervisor as needed to ensure program/contract requirements.
  • Assist Supervisor, department staff and/or other agency personnel as needed to support program operations.

Position Type and Expected Hours of Work:

This is a full-time position. Days and hours of work are generally 9:00 AM to 5 PM Monday - Friday. Additional hours may be required to meet program deadlines, or client needs.

Working conditions and physical demands required:

The demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.

  • Travel for home visits, outreach and/or escorting clients at least 60% of the time using reliable transportation, traversing streets, ascending/descending stairs, and exposed to outside weather conditions.
  • Assess and work in varied home environmental conditions, possibly including vermin/bedbugs.
  • Traverse neighborhoods that may require a high level of alertness and awareness of surroundings.
  • Work with clients who require consistent and comprehensive support, some under potentially stressful conditions.
  • Remain in a stationary position at a workstation and use a computer approximately 40% of the time.
  • Frequently sit for long periods attending meetings.

Qualifications

Education and/or experience required:

  • Bachelor’s degree required or four years of experience in the social services field.
  • Second language required; Spanish or French preferred.

Skills, Licenses, and/or competencies required:

  • Excellent interpersonal skills; the ability to work well independently and as a part of a team.
  • Knowledge of social service delivery systems and local resources.
  • Excellent organizational, time-management, and communication skills.
